JOB SUMMARY We are seeking a dedicated and compassionate
Outreach Family Support Worker to join our Early Help service within Children and Young People’s Services. This is a rewarding opportunity to make a real difference to the lives of children, young people, and families by providing high-quality, targeted outreach and home-based support using a whole family approach.
You will take whole-case responsibility for complex Early Help cases (Levels 2–3), including those stepping down from Children’s Social Care, and work closely with families to reduce risk, build resilience, and improve outcomes. The role involves working with children and young people aged
0–25, including those with SEND needs in line with the SEND Code of Practice.
JOB DESCRIPTION The duties and responsibilities of the role will include:
- Act as Lead Professional for complex Early Help cases, completing assessments and coordinating Team Around the Family (TAF) plans.
- Provide intensive, targeted support to vulnerable children, young people, and families, including those with complex emotional and mental health needs.
- Support smooth transitions between Early Help and Social Care, including cases escalating to Level 4.
- Assess risk and need, formulate outcome-focused plans, and review progress within required timescales.
- Work in line with the Signs of Safety model and use tools such as the Family Outcomes Star to evidence progress.
- Deliver practical parenting support and facilitate family meetings to strengthen relationships.
- Undertake home visits and deliver outreach support across the city.
- Maintain accurate and timely records on electronic case management systems.
- Promote safeguarding, equality, diversity, and anti-discriminatory practice in all aspects of work.
- Attend supervision, training, meetings, and contribute to service development and continuous improvement
THE SUCCESSFUL CANDIDATE The successful candidate will have the following skills and competencies:

- You must be able to work flexibly and travel across the city as required.
