JOB SUMMARY
You will undertake specialist assessments for Deaf, Blind, and Hard of Hearing adults, working in a strengths-based, person-centred way to identify needs, aspirations, and outcomes that promote independence and wellbeing. Residents will always be at the heart of your practice, supported through creative problem-solving, emotional support, and practical interventions.
This role plays a vital part in preventing harm, reducing dependency, and ensuring adults with hearing loss and visual impairment can live safely and independently within their community.

  
JOB DESCRIPTION
The duties and responsibilities of the role will include:

• Carry out specialist assessments for adults with hearing loss, visual impairment, and dual sensory loss
• Design, implement, monitor, and review person-centred care and support plans, including the use of direct payments
• Provide and demonstrate appropriate specialist equipment and assistive technology
• Act as an advocate for Deaf, Blind, and Hard of Hearing residents, promoting accessibility, equality, and inclusion
• Work collaboratively with Audiology services, health professionals, housing, and partner agencies
• Undertake joint assessments and contribute to multi-disciplinary working
• Identify safeguarding concerns and contribute to safeguarding enquiries where required
• Maintain accurate, confidential, and high-quality case records using the Mosaic system
• Raise awareness of sensory impairment across the Borough and advise colleagues accordingly
• Support community projects, outreach work, and group initiatives as agreed with management
• Ensure compliance with the Care Act, Mental Capacity Act, Data Protection legislation, and Council policies
• Contribute to team meetings, supervision, performance appraisals, and continuous professional development
• Ensure all practice aligns with the Council’s commitment to equality, inclusion, and its climate and ecological responsibilities

  
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:

• British Sign Language (BSL) Level 2
• Good general education (A-Level or NVQ Level 3 or equivalent)
• Strong understanding of strength-based practice, independent living, and person-centred approaches
• Sound knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding adults practice
• Experience in assessment, care planning, and reviewing support plans within personal budget frameworks
• Ability to communicate effectively with residents, carers, families, and professionals
• Excellent written skills with the ability to produce clear, accessible, high-quality documentation
• Strong problem-solving skills and ability to work independently using initiative
• Confidence working in multi-agency and multidisciplinary environments
• Competent IT skills and willingness to learn council systems
• Commitment to equality, diversity, confidentiality, and high standards of customer care
