JOB SUMMARY
Seeking an experienced Social Worker to join Reablement Team, a dynamic, integrated service delivering fast-paced responses to hospital discharges, alongside reablement and short-term social care support.
Our service is built around a clear ethos: promoting independence, building resilience, and supporting people to live safely in their own homes. You’ll work closely with colleagues across intermediate care services, hospitals, and community partners to ensure timely, person-centred interventions.
This is a rewarding opportunity for a Social Worker who thrives in a community-based, multidisciplinary environment and is passionate about enabling positive outcomes for adults with physical disabilities and older people.



JOB DESCRIPTION

• Undertaking Care Act Assessments and reviews in residents’ homes
• Delivering short-term reablement interventions to maximise independence
• Supporting safe and timely hospital discharges
• Working collaboratively with Occupational Therapists, Care Assessors, OT Assistants, and partner agencies
• Managing a varied caseload, prioritising effectively in a fast-paced service
• Promoting strengths-based, person-centred practice

  
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:

• Qualified Social Worker with relevant professional registration
• Experience working with adults with physical disabilities and/or older people
• Background in Adult Social Care, Intermediate Care, Home Care, or Reablement services
• Strong communication and relationship-building skills
• Confident working within a multidisciplinary team
• Excellent organisational skills, including caseload management and time management
• A genuine commitment to empowering residents and promoting independence
