JOB SUMMARY
The Support Worker will work as part of Day service, delivering high-quality, person-centred and strengths-based care and support to adults aged 18+ with a range of needs, including ; Learning disabilities, Physical and sensory disabilities, Autism, Mental health needs, Older people, including early onset dementia


JOB DESCRIPTION
The duties and responsibilities of the role will include:

• Care & Support Delivery
  • Deliver personalised care and support in line with individual Support Plans
  • Support with personal care, domestic tasks, independent living skills, and community access
  • Provide care in an enabling, strengths-based, and person-centred manner
  • Support people with medication in line with Support Plans, service policy, and NICE Guideline NG67
  • Support nutritional needs and special dietary requirements
  Safeguarding & Risk
  • Identify and report safeguarding concerns immediately in line with Council policies
  • Apply the Care Act 2014 and Making Safeguarding Personal principles
  • Enable positive risk-taking while promoting safety and independence
  Working in Partnership
  • Work collaboratively with social workers, health professionals, carers, families, and community organisations
  • Attend and contribute to multi-disciplinary meetings
  • Act as key worker for a small caseload of residents
  Recording & Administration
  • Maintain accurate, timely, and confidential written and electronic records
  • Use IT systems effectively to record progress toward outcomes and goals
  • Manage service users’ personal property in accordance with Council procedures
  Professional Responsibilities
  • Maintain high standards of infection control and health and safety
  • Participate in training, supervision, and appraisal
  • Meet agreed service outcomes and personal objectives
  • Uphold the Council’s Dignity for All policy and CARE values
  • Ensure compliance with GDPR, Health & Safety, and relevant legislation
  General
  • Work flexibly across Provider Services as required
  • Undertake duties appropriate to the grade of the post

  
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:

• Qualifications (Essential)
  • Level 2 NVQ or QCF Level 3 (or equivalent), or willingness to work towards
  • Willingness to undertake specialist training (e.g. autism, dementia, sensory awareness)
  Experience (Essential / Preferred)
  • Experience supporting adults with complex needs and behaviours that may challenge
  • Experience working with people from diverse and Black and Minority Ethnic communities
  • Understanding and application of strengths-based approaches
  • Experience delivering activities and group sessions (preferred)
  Skills & Knowledge
  • Knowledge of safeguarding responsibilities and Making Safeguarding Personal
  • Understanding of relevant legislation including the Care Act 2014 and Mental Capacity Act 2007
  • Ability to work independently and as part of a multi-disciplinary team
  • Strong communication skills, including with people who use non-verbal communication
  • Ability to promote independence through daily living skills support
  • Competence in record-keeping and use of IT systems
  • Ability to support medication management in line with policy
  • Safe moving and handling skills and use of specialist equipment
