Are you eager to land a brand new role? Our client, based in the West Midlands, are looking for an Adult Social Worker to work within a Hospital Discharge team. 

Position: Adults Social Worker
Rate: £31.97 p/h (Umbrella) 
Contract Duration: 5 months with Possible Extension 
Essentials: 
- Confident Adult Social Worker with at least 2 years experience
- Enhanced DBS required for this position
- Registered with SWE 
- Social Work Qualification

Purpose:

• Assess the needs of vulnerable adults, promoting community development and support to achieve optimum level of independence when facilitating hospital discharge plans.

Job Responsibilities:

• Assess individual needs and any factors that may impact recovery, developing comprehensive discharge plans.
• Support with community resources such as housing. financial support or mental health services. 
• Provide information to families and carers, to ensure the discharge plan is understood in line with best interests for vulnerable service users.
• Mental Capacity and Safeguarding reviews, to ensure individuals are protected from harm after successful hospital discharge plans have been facilitated.
