Are you eager to land a brand new role? Our client, based in South East London , are looking for an Adult Social Worker to work within Hospital Discharge.

Position: Social Worker - Hospital Discharge x3
Rate: £28 p/h (Umbrella) 
Contract Duration: 3 months with Possible Extension 
Essentials: 
- Confident Adult Social Worker with at least 2 years experience
- Enhanced DBS required for this position
- Registered with SWE 
- Social Work Qualification

Purpose:

• 2x Social Workers to support and facilitate hospital discharges.
• 1x Social Worker to support discharged patients who are awaiting care provision from Reablement services.

Job Responsibilities:

• Discharge planning within the community, working alongside Multi Disciplinary Team.
• Complete relevant assessments, safeguarding enquiries, reports and care packages in a timely accurate manner.
• Work alongside Care Act and Mental Capacity act to make best interests where required

Job Requirements:

• 2 years experience, within a Hospital Setting and Adult Social Care.
• Experience of Multi Disciplinary Working.
• Knowledge of Care Act, Mental Capacity and Safeguarding.
