High-Level Overview for Candidates
TAPP Workforce Solutions is seeking a Nurse Practitioner to provide comprehensive primary care services within a community clinic that supports Long-Term Care (LTC) and geriatric populations in Scarborough.
This role focuses on delivering primary care to older adults and LTC residents — many of whom present with complex chronic conditions and mental health needs. You will work collaboratively within an inter-professional healthcare team, providing assessment, treatment, and ongoing management for a broad range of medical conditions.
The LTC home operates within a community clinic setting, integrating primary care with geriatric and mental health components. This position also supports a recently implemented Mobile Health Clinic (Healthcare on Wheels), extending care directly into the community via a mobile van and other health hubs.
Key Responsibilities
- Provide assessment, diagnosis, treatment, and follow-up care for acute and chronic illnesses
- Deliver primary care services to LTC residents and geriatric patients
- Manage complex chronic conditions and age-related health concerns
- Provide care for patients presenting with mental health concerns commonly associated with geriatric populations
- Conduct comprehensive health assessments and develop individualized, patient-centered care plans
- Provide medication management and monitoring
- Support patients and families through education, care planning, and treatment management
- Participate in peer/chart audits and quality improvement initiatives
- Work collaboratively with physicians, nurses, and allied health professionals within an inter-professional care team
- Provide telephone consultation and follow-up care as required
Requirements Education - Master’s prepared Nurse Practitioner with Primary Health Care Certificate
Certifications - Registration in good standing with the College of Nurses of Ontario – Extended Class
- Member of NPAO with professional liability coverage
Experience - 3+ years of Nurse Practitioner practice experience
- Experience in primary care strongly preferred
- Experience in long-term care, geriatrics, or senior care settings preferred
- Experience working with mental health and addiction populations considered an asset
Technical Skills - Demonstrated ability to work independently with strong clinical decision-making skills
- Excellent communication and interpersonal skills
- Strong organizational and care coordination abilities
- Valid driver’s license and access to a vehicle required
Candidate Fit This role is ideal for a self-directed NP with a genuine passion for geriatric and community-based care. The right candidate is comfortable working across multiple care settings — including mobile and community health environments — and brings strong clinical judgment, compassion for complex aging populations, and a collaborative spirit within an inter-professional team. Comfort with flexibility in work locations and schedule is essential.
