Key Responsibilities
- Provide comprehensive assessment, intervention, monitoring, and follow-up for patients with acute, chronic, mental health, and addiction conditions
- Conduct client interviews and physical examinations, and develop evidence-based, client-centered treatment plans
- Deliver primary care services with a focus on preventive care and long-term health maintenance
- Participate actively within an inter-professional model of care
- Provide patient and family education on treatment administration, side effect management, and follow-up planning
- Support community education sessions and participate in peer and chart audit reviews
- Offer telephone support and act as a clinical resource for your healthcare team
- Mentor and precept NP students and medical students as opportunities arise
Requirements Education - Master’s-prepared Nurse Practitioner with a Primary Health Care certificate
Certifications - Current registration in good standing with the College of Nurses of Ontario — Extended Class (NP-PHC or equivalent)
- Active NPAO membership with additional professional liability coverage
Experience - Minimum 3 years of NP practice experience, including advanced nursing assessment in an inter-professional setting
- Demonstrated experience serving marginalized populations living with chronic illness, mental health challenges, and addiction
- Demonstrated ability to work independently with a high degree of self-direction and strong organizational skills
- Prior NP experience in a community health setting is strongly preferred
- Pediatric experience is considered a strong asset
Technical Skills - Advanced nursing assessment skills including physical examination and diagnostic reasoning
- Evidence-based treatment planning and clinical decision-making across acute and chronic conditions
- Proficiency with electronic medical records and community health documentation systems
