TAPP Workforce Solutions is recruiting a Primary Care Nurse Practitioner on behalf of a community health organization serving diverse populations in Scarborough. This role focuses on delivering comprehensive, family-centered primary care to unattached patients, with an emphasis on health promotion, disease prevention, and equitable access to care.
Key Responsibilities
- Provide comprehensive primary care across the lifespan within NP scope
- Assess, diagnose, and manage acute and chronic conditions
- Deliver preventive care, including immunizations
- Develop and implement patient-centered care plans
- Collaborate within an interdisciplinary team
- Support marginalized populations with complex health needs
- Participate in health promotion and outreach initiatives
- Maintain clinical documentation and ensure confidentiality
- Contribute to quality improvement initiatives
- Participate in on-call rotation and occasional home visits
Requirements Education - Master’s degree in Nursing
Certifications - Primary Health Care Nurse Practitioner (PHCNP) certification
- Registered in good standing with the College of Nurses of Ontario (Extended Class)
Experience - Experience working with diverse and underserved populations
- Experience in community health or primary care settings (preferred)
Interview Process - Stage 1: Initial screening call with TAPP Recruiter (20–30 min)

- Stage 2: Credential verification and reference checks
