Care Manager - Halifax

The Role
The successful candidate will play a key role in supporting the effective day-to-day operation of the branch, ensuring high-quality, person-centred care delivery and compliance with regulatory standards.

Key Responsibilities:

• Allocate care workers to customer visits using effective rostering systems, ensuring continuity of care while considering location, skills, and client preferences
• Manage new referrals from private clients and local authority teams, ensuring all information is gathered and appropriate care planning is completed
• Support the Branch Manager in identifying and developing new business opportunities
• Match care workers to clients, ensuring all assignment conditions are clearly communicated, including rates of pay
• Maintain accurate records and contribute to the smooth operation of the office
• Communicate effectively with care staff, clients, and external stakeholders
• Use IT systems confidently to manage rotas, care records, and reporting

About You:

• 3-6 months' experience as a Care Coordinator in a domiciliary care setting
• Minimum 12 months' domiciliary care experience within the last 3 years
• NVQ Level 3 in Health & Social Care and working towards Level 4
  (or willingness to enrol upon appointment)
• Full UK driving licence and access to your own vehicle
• Willingness to work every other weekend as on-call support

Desirable

• Knowledge of PASS and Care Planning (CP) systems
• Good ICT knowledge
• Understanding of the CQC 5 Key Lines of Enquiry (KLOEs)
