He kōrero whakamārama mō tēnei tūranga | About the role   
You will play a crucial role in ensuring that patients receive comprehensive and coordinated care, which is especially important for those with chronic or complex health conditions. This includes scheduling and managing appointments with various healthcare providers to ensure timely and appropriate care.  
   
Takohanga | Responsibilities   

• Manage rostering and scheduling for complex care clients 
• Support and train complex care teams 
• Ensure training compliance and sector standards 
• Build strong relationships with clients and support teams 

Ngā tohungatanga mō tēnei tūranga | Skills and experience  

• Strong planning, computer and caseload management skills 
• Excellent communication skills 
• Cultural awareness and sensitivity 
• Full, current NZ driver’s license 
• Permanent NZ work rights 
• Monday–Friday, 8:00am–4:30pm office based + client visits
