Community Clinical Care Manager | RN Leadership Role

A well-established Brisbane community provider is now seeking an experienced RN to step into an exciting Clinical Care Manager role. This position offers a strong mix of clinical oversight, team support and service development within a collaborative, forward-thinking environment.

You’ll play a key role in strengthening clinical practice, guiding care staff and enhancing client outcomes, with a great balance of office and field work. Backed by supportive leadership and ongoing service growth, this is a fantastic opportunity to step into a role with real influence, progression and purpose.

Role & Benefits:

• $100k–$110k + super (negotiable based on experience)
• Full-time or 4 days per week (pro rata)

Key Duties:

• Conducting clinical reviews, assessments and person-centred care planning
• Providing mentoring, coaching and daily support to Care Coordinators and care staff
• Strengthening documentation, governance, compliance and quality standards
• Supporting incident reviews, service improvements and internal clinical processes
• Building strong relationships with clients, families, GPs and allied health
• Ensuring safe, effective, high-quality community care delivery
• Assisting with systems development and workflow updates

About You: 

• AHPRA Registered Nurse
• 3+ years in community aged care 
• Experience supporting or leading staff is highly advantageous
• Confident with clinical assessments, documentation and risk management
• Enjoys a mix of office-based oversight and community-based client visits
• Strong communicator who builds trust easily
• Interested in contributing to quality, governance and service improvement
• Tech-savvy
