Care Coordinator - Community Aged Care | Hybrid | NFP Benefits

This is a well-supported community role offering flexibility, autonomy and the chance to build strong relationships with your clients.

You’ll manage a caseload, lead assessments and care planning, coordinate services and support clients to live safely and independently at home.

About the Role

• Complete client visits, comprehensive assessments, reviews and goal-based care planning
• Coordinate services, manage referrals and oversee new client commencements
• Manage Support at Home funding and budgets across an active community portfolio
• Work closely with nursing, allied health and operations teams to maintain high standards
• Monitor changes in client needs and proactively adjust supports
• Maintain compliance with aged care legislation, quality frameworks and internal processes

What We’re Looking For

• Minimum 2 years’ experience in community aged care coordination (Care Coordinator / Case Manager roles)
• Strong communication and stakeholder management skills
• Excellent organisation and prioritisation ability
• Proactive, client-first approach
• Knowledge of aged care compliance, quality standards and care planning
• Current Australian driver’s licence
