Community Registered Nurse - Brisbane

A leading community aged care provider is expanding and has an opportunity for an experienced Registered Nurse to join their Brisbane based team. This role suits an RN who wants autonomy, a balanced caseload, strong clinical support and the chance to make a genuine impact in the community.

What You’ll Be Doing:

• Provide clinical support to clients accessing HCP/CHSP services
• Complete assessments, reviews and care planning
• Provide wound care, medication management and chronic disease management
• Collaborate with Care Partners and ENs to coordinate client needs
• Maintain accurate, same-day documentation via AlayaCare
• Mentor and support ENs within your region
• Participate in weekly clinical meetings, reviews and training
• Build strong relationships with clients and their families

What You Bring:

• Current AHPRA registration
• Minimum 1 year RN experience within community aged care
• Strong assessment and wound care capability
• Confidence guiding ENs or junior nurses
• Understanding of community care and aged care frameworks
• Current driver’s licence
