Clinical Care Manager - Community Aged Care

A well-established and growing Brisbane community aged care provider is seeking an experienced Registered Nurse to step into an exciting Clinical Care Manager role.

This position offers a genuine balance of clinical oversight, team leadership and service development, within a collaborative, forward-thinking environment that values quality, relationships and continuous improvement.

You’ll play a key role in strengthening clinical practice, supporting Care Coordinators and care staff, and improving outcomes for clients living independently in the community. With strong leadership support and ongoing organisational growth, this is a role where you can truly influence, progress and make an impact.

Role & Benefits

• $110k – $115k + super (negotiable based on experience)
• Full-time or 4 days per week (pro rata)

Key Responsibilities

• Conduct clinical reviews, assessments and person-centred care planning
• Provide mentoring, coaching and day-to-day support to Care Coordinators and care staff
• Strengthen documentation, clinical governance, compliance and quality standards
• Support incident reviews, continuous improvement and internal clinical processes
• Build strong relationships with clients, families, GPs and allied health
• Ensure safe, effective and high-quality community care delivery
• Assist with systems development and workflow improvements

About You

• AHPRA-registered Registered Nurse
• 3+ years’ experience in community aged care
• Experience supporting or leading staff is highly regarded
• Confident in clinical assessments, documentation and risk management
• Enjoys a mix of office oversight and community-based work
• Strong communicator who builds trust easily
• Interested in quality, governance and service improvement
• Tech-savvy and comfortable working across systems
