Are you an experienced Adult Social Care practitioner ready for your next opportunity? Our client is seeking a Senior Practitioner Social Worker to support adults with a range of care and support needs.

Position: Senior Practitioner – Adults 
Location: London

Purpose of the Role

To provide social work support to adults, helping assess their needs, manage risk and promote independence and wellbeing.

Job Responsibilities

• Assess adults' needs and identify appropriate support.
• Work with individuals and relevant professionals to plan support.
• Help manage risk and promote positive outcomes.
• Maintain accurate records and contribute to case decisions.

Job Requirements

• Relevant social work qualification and professional registration.
• Experience in Adult Social Care.
• Strong assessment, care planning and risk management skills.
• Ability to work effectively with adults and partner agencies.
