Position: Care Assessor – Adult Social Care
Location: East London

Purpose of the Role
To support adults with care and support assessments, helping identify their needs and the services required to promote independence and wellbeing.

Job Responsibilities

• Support the assessment of adults' care and support needs.
• Help identify appropriate support options based on individual circumstances.
• Maintain accurate records and assessment information.
• Work with relevant professionals to support positive outcomes.

Job Requirements

• Relevant experience in adult social care assessment.
• Knowledge of assessment processes and care planning.
• Good communication and record-keeping skills.
• Ability to work with adults with a range of care and support needs.
