Are you an experienced Adult Social Worker looking for your next opportunity? Our client is looking for a Social Worker to join their Community Based service in the Southwest Region.

Position: Social Worker – Community Based
Rate: £32 per hour
Contract Duration: 3-6 Months
Purpose:

• You’ll support adults with care and support needs through assessment, reviews, safeguarding and care planning within a community-based service.

Job Responsibilities:

• Complete Care Act assessments, reviews, Section 42 enquiries and Mental Capacity Assessments.
• Undertake Best Interest Decisions and support hospital discharge where required.
• Manage work relating to out-of-area placements and participate in the duty rota.
• Complete Court of Protection work where required and maintain accurate records.

Job Requirements:

• Previous Local Authority Adult Social Care experience is essential. Please do not apply if you have not worked within a Local Authority setting.
• Social Work qualification with current Social Work England registration.
• Experience with Care Act assessments, safeguarding and Mental Capacity work.
• Experience using LAS is preferable.
