Are you an experienced Care Assessor looking for a new interim opportunity? Our client is looking for Care Assessors to join their Adult Social Care service in Northwest London.

Position: Care Assessor
Rate: £22.41 p/h
Contract Duration: 3-6 Months

Purpose:

• You’ll support Adult Social Care by assessing people's needs and helping to identify appropriate care and support.

Job Responsibilities:

• Complete assessments and reviews of adults' care and support needs.
• Work with individuals, families and partner agencies to identify appropriate support.
• Maintain accurate records and ensure assessments are completed within required timescales.
• Contribute to person-centred care planning and positive outcomes.

Job Requirements:

• Previous Local Authority Adult Social Care experience is essential. Please do not apply if you have not worked within a Local Authority setting.
• Previous experience completing care assessments and reviews.
• Good understanding of strengths-based and person-centred practice.
• Strong communication, assessment and case recording skills.
