Are you looking for your next Social Work opportunity? Our client, based in the Southeast Region, are looking for a Hospital Discharge Social Worker to join their hospital social work team.

Position: Hospital Discharge Social Worker
Location: St Richard’s Hospital, West Sussex
Pay Rate: £38 per hour Umbrella OR £35 per hour + £150 weekly expenses
Contract Duration: Temporary
Working Arrangements: Hybrid – 2–3 days per week on site
Essentials:

• Social Work qualification
• Registered with SWE
• Experience within Adult Social Care
• Experience working within hospital or discharge services

Purpose:
To assess people in hospital and following discharge, arranging appropriate care and support to promote independence and positive outcomes.
Job Responsibilities:

• Complete strengths-based assessments and develop effective support plans
• Support hospital discharge and help people regain independence
• Work closely with families, carers, health professionals and community services
• Contribute to safeguarding enquiries and risk management

Job Requirements:

• Experience completing Adult Social Care assessments
• Knowledge of relevant legislation and strengths-based practice
• Ability to manage a varied and fast-paced caseload
• Strong communication, organisation and record-keeping skills
