Office Based/Hybrid: Remote but may need to attend schools if requested
DBS Required: Enhanced
Rate of Pay: £300 per day
Pay Negotiable: No

Short Brief/Requirement:  
The Assessment Coordinator will act as the key point of contact on behalf of the local authority, for children and young person (0-25) with special educational needs and disability and their families. 

Key Responsibilities:

• In line with the SEND Code of Practice and Children and Families Act they will coordinate any activities that are required to assess or review needs, meet any outcomes specified, writing and editing Education, Health and Care Plans and to  coordinate educational placements to ensure the provision of holistic care and support.
• They can work from home but they may be required to come into the office for training but also Schools and Families may request an in person Draft EHCP meeting. Many Schools request this virtually but some do prefer in person.
• They will be writing the EHCP Plans and also case working with the families and schools after.
• Be proactive in promoting multi agency partnership working as best practice. To be instrumental in developing pathways for joint working. Coordinate the development of ‘Person Centred’ meetings to ensure that the child/young person and their parents/carers voice, hopes and ambitions are clearly heard and represented.
• Carry out all tasks and responsibilities in accordance with legal requirements across education, health and social care, including identifying if a vulnerable child or young person may be at risk.
• Contribute to the development of SEND & Inclusion Services, working in partnership with Local Area colleagues and to respond to operational needs by initiating, developing and implementing innovative solutions.

Job Requirements:

• Previous experience working within a local authority or a similar organisation.
• At least 2 years experience within a Send Assessment Coordinator role or equivalent.
• Enhanced DBS
