Tempting Recruitment are working alongside our client, a Local Authority, who are looking to recruit an experienced  Social Worker to join their team based in Southwest England 
   
JOB DETAILS
PAY RATE: £25 -£28 per hour  
WORK TYPE: Temporary
WORK PATTERN: Full Time
LOCATION: Southwest England 
   
JOB SUMMARY
The Social Worker to work within a Triage and Response Team, integrated within the discharge teams. 
  
JOB DESCRIPTION
The duties and responsibilities of the role will include:

• Supports people to be discharged from hospital, when safe and appropriate to do so, and continuing their care and assessment out of hospital  

  
THE SUCCESSFUL CANDIDATE
The successful candidate will have the following skills and competencies:

• Relevant Qualifications, with a registered body in Social Work, Occupational Therapy, Nursing and experience of working in a Health or Social Care setting
• Experience of working in a Hospital team.
