Kaiarataki - Cancer Care Navigator

Turuki Health Care is a kaupapa Maaori organisation delivering integrated health, wellbeing and social services across Taamaki Makaurau. Our mission is to provide high quality, accessible and affordable healthcare to whaanau, with a dedicated focus on equity and culturally responsive care.
  
We are looking for a compassionate and community focused Kaiarataki - Cancer Care Navigator to join our new Cancer Care Coordination Service. In this role you will work alongside whaanau across Auckland to support them through their cancer journey, helping them understand their diagnosis, navigate the health system and access the services they need. This role is fulltime, Monday to Friday, 8.30am to 5pm.

This is an opportunity to work directly with whaanau and support them to access care and overcome barriers during cancer treatment and recovery.
  
Your mahi will include:

• Connecting with whaanau referred to the Cancer Care Coordination Service
• Supporting whaanau to understand their diagnosis, treatment, and care pathways
• Helping whaanau attend appointments and move through treatment and recovery
• Completing needs assessments and identifying supports required
• Connecting and advocating for whaanau with health, social and community services
• Coordinating practical supports such as transport and accommodation
• Supporting whaanau to stay engaged with treatment and services
• Maintaining case notes, data collection and reporting requirements
• Building strong relationships with internal teams and external providers to support referral pathways

You are passionate about supporting whaanau and improving health outcomes in your community. You enjoy building relationships and working alongside people to find practical solutions.
  
Ideally you will bring:

• Experience in health, community or social services (e.g. kaiaawhina, navigator, community worker, social worker)
• A good level of communication and relationship-building skills
• Ability to engage effectively with whaanau and community partners
• Good organisational and computer skills, with the ability to learn new systems
• Ability to manage a caseload and prioritise your workload
• Understanding of kaupapa Maaori and whaanau-centred approaches
• Commitment to improving Maaori health outcomes
• Emotional resilience and ability to support whaanau through challenging situations
• Full New Zealand Driver’s Licence
• Eligibility to work in New Zealand

  
Experience working within kaupapa Maaori services or knowledge of health systems is an advantage.
  
The ability to speak Te Reo Maaori or a Pacific language is highly advantageous.
