We’re looking for an experienced and people-focused Home Care Coordination Team Lead to lead our Coordination Team and help ensure clients receive safe, reliable and high-quality home care services.
This is a leadership role for someone who enjoys developing people, improving performance and solving operational challenges. While you’ll maintain a strong understanding of home care coordination, your primary focus will be on leading the team — not managing your own client workload, but working closely alongside the team, providing guidance, support and leadership to help them succeed.
What you’ll be responsible for:
- Lead, motivate and support a team of Care Coordinators.
- Set clear expectations, priorities and performance standards.
- Monitor team KPIs, productivity, service continuity and workforce utilisation.
- Lead team meetings, daily huddles and operational planning.
- Coach team members and support capability, development and succession planning.
- Identify and address performance and capability concerns.
- Oversee workload allocation and workforce coverage.
- Proactively manage vacant visits, staffing risks and service disruptions.
- Act as the first escalation point for complex coordination and service delivery matters.
- Monitor quality, compliance, incidents, complaints and operational risks.
- Use service data and team feedback to identify improvement opportunities.
- Lead approved process improvements and support organisational change.
- Build strong relationships with clients, families, support workers, managers and external stakeholders.
- Promote a culture of accountability, collaboration, customer focus and continuous improvement.
What we’re looking for: You’ll be a confident and capable leader who can balance people leadership with operational accountability.
Ideally, you’ll bring: - Experience leading or supervising a coordination, scheduling or home care team.
- Strong knowledge of home and community care service delivery.
- Excellent communication and relationship-management skills.
- Confidence coaching people and managing performance conversations.
- Strong organisational and problem-solving skills.
- The ability to prioritise competing operational demands.
- A proactive approach to identifying and managing risk.
- Experience working with quality, compliance and service standards.
- A genuine commitment to client outcomes and high-quality care.
- The ability to influence, collaborate and work effectively across teams.
Experience working within the New Zealand health, disability, aged care, ACC or community care sectors will be highly regarded.
