DESCRIPTION:
A well-established and rapidly expanding provider of senior-focused, value-based primary care services is seeking a compassionate and experienced Primary Care Nurse Practitioner (APRN) to join its growing Florida team.

KEY RESPONSIBILITIES: - Provide comprehensive primary care services to senior patients within a value-based care framework
- Manage chronic conditions with a strong emphasis on preventative medicine and quality metrics
- Develop personalized treatment plans focused on improving long-term patient outcomes
- Collaborate with interdisciplinary care teams to optimize patient health
- Maintain accurate and timely documentation within EMR systems
- Build strong, trusting relationships with patients and families
- Support organizational quality initiatives and performance benchmarks
REQUIREMENTS: - Active and unrestricted Florida APRN license
- DEA license or ability to obtain within 2 months of hire
- 2-3+ years of experience in primary care and/or value-based care environments
- Experience working with Medicare Advantage, Medicare, and Medicaid populations
- Strong knowledge of chronic disease management and preventative care models
- Familiarity with quality metrics within value-based reimbursement structures
- Proficiency in EMR systems (eClinicalWorks experience preferred)
- Excellent documentation and patient communication skills
- Demonstrated compassion, professionalism, and patient-centered approach
