Role This role offers the opportunity to make a meaningful impact by providing trauma-informed, veteran-centric case management to individuals with complex and multifaceted support needs. You will play a vital role in improving wellbeing outcomes, strengthening client independence, and ensuring coordinated access to services and entitlements. Your duties will include:
Managing a caseload of clients with complex mental health, medical and psychosocial needs.
Conducting intake, risk and wellbeing assessments and developing tailored support plans.
Acting as the primary point of contact to coordinate internal and external support services.
Liaising with stakeholders across government, community organisations and service providers.
Identifying and escalating high-risk or high-profile matters through established protocols.
Preparing high-quality documentation, summaries and briefings to support informed decision-making.
Skills and Experience we are looking for:
Tertiary qualifications in Nursing, Psychology, Social Work, Occupational Therapy or another relevant health discipline.
Demonstrated experience in clinical or complex case management, ideally with clients experiencing mental health conditions or multiple support needs.
Strong understanding of disability, capacity-building models and the social determinants of health and wellbeing.
Excellent verbal and written communication skills, with the ability to build effective relationships with diverse stakeholders.
Experience undertaking triage, assessments and case coordination via telephone.
