Role
This role offers the opportunity to make a meaningful impact by providing trauma-informed, veteran-centric case management to individuals with complex and multifaceted support needs. You will play a vital role in improving wellbeing outcomes, strengthening client independence, and ensuring coordinated access to services and entitlements.
Your duties will include:

• Managing a caseload of clients with complex mental health, medical and psychosocial needs.
• Conducting intake, risk and wellbeing assessments and developing tailored support plans.
• Acting as the primary point of contact to coordinate internal and external support services.
• Liaising with stakeholders across government, community organisations and service providers.
• Identifying and escalating high-risk or high-profile matters through established protocols.
• Preparing high-quality documentation, summaries and briefings to support informed decision-making.

Skills and Experience we are looking for:

• Tertiary qualifications in Nursing, Psychology, Social Work, Occupational Therapy or another relevant health discipline.
• Demonstrated experience in clinical or complex case management, ideally with clients experiencing mental health conditions or multiple support needs.
• Strong understanding of disability, capacity-building models and the social determinants of health and wellbeing.
• Excellent verbal and written communication skills, with the ability to build effective relationships with diverse stakeholders.
• Experience undertaking triage, assessments and case coordination via telephone.
