Comprehensive Primary Care Team - Community Pharmacist

About the Initiative ProCare have launched Comprehensive Primary Care Team (CPCT), a pioneering model aimed at enhancing health equity by expanding service access through innovative roles within existing general practice teams. This initiative is full funded through to end of June 2026 and integrates allied health, social work, and pharmacy professionals into practices to better serve at-risk communities. Funding supports free patient access to essential services, easing pressure on GP teams while improving on-site family-focused care. It also invests in workforce development, enabling professionals to pursue specialized pathways. We invite expressions of interest for Care coordinators, Pharmacists or Paramedics eager to join the frontline of community care.
About the Role
The Practice Pharmacist will act as the subject matter expert within the practice on all matters relating to medicines information and advice, directly seeing patients to offer individualized pharmacotherapy advice and offering education and pain management sessions to patients/ whanau while improving awareness of medicine related harms, amongst other duties.
As a CPCT Practice-based Pharmacist some of your duties will include:
- Medicines management, optimization, and reconciliation
- Clinical evaluations and effectiveness of best practice
- Medication evaluation, audit, and quality improvement to improve best practice and equitable access
- Development and management of standards, policies, and procedures of safe, effective, and equitable access and use
- Actively supporting and working within an IDT structure and using systems and processes that enhance IDT functioning
- Developing and strengthening partnerships with other health and wellbeing providers
We are interested in hearing from Pharmacists with a range of experience levels and background at this stage, however our successful applicants will typically need to have:
- A current APC with no restrictions
- 2 years or more experience as a registered pharmacist in primary or secondary care facilities
- Achieved or actively interested in pursuing post-grad studies in clinical pharmacy or pharmacist prescribing.
- Comprehensive understanding of inequities in access and health outcomes in Aotearoa, New Zealand and detailed knowledge of Te Tiriti o Waitangi and its application to shaping all efforts within primary care
- Strong cultural competency and confidence with sufficient knowledge of the peoples, languages and cultures that form our most at-risk communities
- Demonstrable background working in a collaborative, mutli-disciplinary health team environment would be advantageous.
- Experience supporting management of long-term conditions through initiation and continuation of prescribed treatments, will be highly regarded.
