ProCare have launched Comprehensive Primary Care Team (CPCT), a pioneering model aimed at enhancing health equity by expanding service access through innovative roles within existing general practice teams.
This initiative is full funded through to end of June 2026 and integrates allied health, social work, and pharmacy professionals into practices to better serve at-risk communities. Funding supports free patient access to essential services, easing pressure on GP teams while improving on-site family-focused care.
It also invests in workforce development, enabling professionals to pursue specialised pathways. We invite expressions of interest for Care coordinators, Pharmacists or Paramedics eager to join the frontline of community care.

- Whangarei practice – 30 hours per week
About the Role As an Integrated Health Practitioner, you will be a cornerstone of our inter-disciplinary team, delivering high-quality, culturally responsive care to individuals and whānau with complex health needs. Your multifaceted responsibilities will include:
Pharmacist Duties - Provide expert medicines information and pharmacotherapy advice directly to patients.
- Lead education and pain management sessions for patients and whānau.
- Promote safe medicine use and awareness of medicine-related harms.
Care Coordination Duties - Develop and implement personalised care plans in collaboration with multidisciplinary teams. during in person and online consultations.
- Proactively identify patients at risk of medication related harm or with suboptimal therapy and work with patients and other clinicians in the practice to reduce or ameliorate these risks.
- Participate in non-patient facing quality improvement activities in the practice to promote overall better health outcomes for the population of patients enrolled at the practice.
Paramedic Duties - Conduct front-line assessments via telehealth and in-person consultations.
- Perform telephone triage, wound care, suturing, and issue medications under standing orders.
- Order lab tests and imaging (under GP supervision).
- Prioritise care to support hospital avoidance and early discharge.
We’re Looking For - Pharmacists need a valid APC, be enrolled in Post Grad Certificate in Clinical Pharmacy or higher.
- Paramedics need a valid APC, be enrolled on Extended Care Paramedic qualification.
- Care Coordinator needs a valid APC as a nurse or allied health professional.
- Proven experience in primary care or community health settings.
- Strong communication and coordination skills.
- Commitment to equity, cultural safety, and Te Tiriti o Waitangi principles
