We are seeking a Support at Home Clinical Care Partner to work in a part time (60 hours/fortnight) capacity either at Nhill, Natimuk, Minyip or Dimboola.
ABOUT THE ROLE
The primary purpose of this position is to coordinate and ensure the effective delivery of services to WWHS Support at Home participants. These participants have been assessed through the Single Assessment System and funded to implement a support plan.
KEY SELECTION CRITERIA
Essential:
Have effective communication and organisational skills.
Professional qualifications as Registered Nurse, Allied Health Practitioner or relevant community services field, with post graduate experience in aged care including community-based experience.
Well-developed interpersonal skills, effective communication and organisational skills.
Demonstrate the ability to work well as part of a team and contribute to a positive team environment and culture.
Have experience in using technology to support effective documentation, communication and service delivery – for example, client management system software.
Demonstrate an ongoing commitment to professional development.
Desirable:
Have previous experience working in a similar position or field of work, with demonstrated case management skills and experience.
PRE-REQUISITES:
NDIS Worker Screening Clearance.
Working with Children Check.
Fully vaccinated (including influenza) as per current Department of Health Directives.
Australian Driver’s License.
KEY RESPONSBILITIES
Develop a care plan for WWHS Support at Home participants consistent with their aged care assessment.
Work with the participant to determine how those services will be delivered, involving registered supporters, informal carers and family members in the discussions with consent of the participant.
Develop the Support at Home Agreement, including care plan and budget as per the Support at Home program manual.
Coordinate the provision of services for each participant as per the care plan, including referral to external services for assessment and services.
Maintain regular contact with the participant and their registered supporters, including ongoing monitoring, reassessment and review of the care plan when needs change and in line with the strengthened Quality Standards.
Support participants with referral for review by the Single Assessment System via My Aged Care if needs significantly change.
To be part of the community care rotating after-hours on-call phone roster.
