About the role:
The Community Registered Nurse will provide clinical support and guidance across sites in South East Queensland, supporting the development, review, and maintenance of clinical care plans, health assessments, and client supports. The role includes providing clinical advice and training to frontline staff, using evidence‑based practice and a person‑centred approach to promote safe, high‑quality care and optimal health outcomes for clients.
This is a part‑time position, with hours to be negotiated and flexible within standard business hours, based in Brisbane for a 6‑month parental leave cover contract. Role Responsibilities:
- Deliver appropriate health care support and services including health care planning, medication management and various high intensity personal supports in keeping with the NDIS Practice Standards - High Intensity Support Skills Descriptors.
- Support Service Managers/Coordinators and teams to coordinate and review the delivery of clinical support services to assist clients in implementing Health Care Plans and to remain safe and healthy within their assessed capacity or in conjunction with guardians/persons responsible who have decision-making authority.
- Provide advice and recommendations regarding intake and onboarding of new clients by reviewing health care ensure documentation and updates required to ensure a smooth transition to Achieve Australia.
- Monitor the implementation of client Health Care Plans to assess if associated complex client issues require amendments to health care objectives.
- Provide appropriate and timely attention when informed of a client hospitalisation and discharge, providing support to staff by explaining identified recommendations and required actions to be taken in keeping with organisation policy and procedures.
- Provide care staff with advice, coaching, training in client health-related plans and feedback to influence the improvement of quality health care outcomes for clients.
- Liaise with health professionals, and communicate advice and recommendations to clients, staff, medical professionals, and other stakeholders.
- Develop and revise individualised client health protocols in keeping with client needs and organisation policy and procedure to keep pace with current clinical best practice and communicate any changes to clients, persons responsible, staff and key stakeholders
- Build and maintain strong internal and external stakeholder relationships including clients and families and carers to address practical issues, enhance operational efficiency and meet client health care requirements.
To be successful in this role, you will need: - Tertiary qualifications or equivalent including holding a current Registered Nurse (RN) registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum of 3 years recent postgraduate nursing experience
- Demonstrated skills and experience in clinical leadership, education, and training
- Evidence of ongoing professional development as reflected in a Professional Practice Portfolio
- Demonstrated ability to use evidence‑based practice and contribute to quality improvement initiatives that enhance clinical care and service outcomes.
- Current QLD Drivers’ License
- Current Working with Children Check and NDIS Workers Screening Check
