Xavier Children's Support Network is part of the Catholic Archdiocese of Brisbane and has been operating in the disability services sector in Brisbane for over 60 years. Xavier provides community nursing services to children and young people and for the older person with complex health and/or disability needs. Services are provided in the client home, in the community, at centre-based locations, in schools and in structured accommodation such as respite services.

This Registered Nurse role will be based in our short term accommodation/ respite facility 'Xavier Place' at Yeerongpilly and will also work across various community locations. You will be part of our team leading clinical and support service teams to support the health and wellbeing of people with complex disability needs and their families throughout the Brisbane region. Your role will include direct care, developing health related plans for children and young adults with complex needs and leading the team to provide the best care possible.
Although the primary focus of the role is supporting children and young people with complex disabilities, adults with complex care and health needs are also supported including aged care needs.
You will achieve this by using evidence-based practices for people living with complex needs to enjoy community life. Health literacy, teaching and supporting people and families to manage an individual’s health risks is also a key aspect of the role. Our goal is to work with individuals and families, other health service providers and our internal teams to help people with complex needs remain living in their homes, interact safely in their community and importantly to prevent avoidable hospital admissions.
Ideally you will have:
- Demonstrated knowledge and skills as a Registered Nurse with complex health and disability needs clients (children, adults and aged care clients) including relevant post graduate experience in medical nursing, preferably with children, in the community (preferred) and/or hospital, aged care settings
- Practical clinical experience with PEG tubes; enteral feeding; suctioning; stomas; swallowing and choking risks and general complex clinical cares such as tracheostomy care is advantageous
- Previous experience in mentoring and training new workers, students and personal carers on complex care needs and care requirements would be advantageous
- Demonstrated skills in assessment and planning of care for individuals with complex health or disability needs
- Demonstrated resilience and openness to change with ability to work within a variety of situations and with individuals and families of diverse backgrounds and experiences
- Strong communication skills (clear and confident communicator, good listening and understanding skills with an ability to effectively influence others)
- Problem solving skills to meet the unique needs of individuals and families in the community
- Collaborative/team approach together with an ability to work independently with limited direct supervision
Other requirements include:
- Bachelor of Health Science - Nursing with AHPRA registration (essential)
- Post grad. study in paediatric or community nursing (desirable)
- Current Australian driver's licence and access to an insured and reliable vehicle
- Ability to meet suitability requirements including an acceptable national police check and an NDIS Worker Screening Check
- Ability to work in varied locations with the potential to travel within the Brisbane Metro area.
- Availability for on-call work (predominantly phone based)
- Australian citizenship, permanent residency or other suitable work rights
- Alignment with the organisation's philosophy and values
