About the Role:
Reporting to the Regional Manager, this role provides leadership, coordination, and operational oversight to ensure the delivery of high-quality, person-centred aged care services. The Coordinator plays a key role in driving service excellence, compliance with the Aged Care Quality Standards, and alignment with Centacare’s Mission, Vision, Values, and Catholic Social Teachings.
This role is responsible for ensuring services are responsive, innovative, cost-effective, and delivered within funding and legislative frameworks. A strong focus is placed on client experience, workforce capability, continuous improvement, and sustainable service delivery across the region.
Key Responsibilities
- Lead regional service delivery planning with the Regional Manager to ensure safe, person-centred, and compliant services.
- Oversee care planning, assessments, reviews, and service delivery to ensure quality standards and regulatory requirements are met.
- Supervise and coach Care Partners and Support Staff Leads to build capability and ensure best-practice service delivery.
- Provide operational guidance and escalation support to staff to ensure consistent and effective service delivery.
- Oversee Hub and Cottage Respite services within the region.
- Build and maintain strong stakeholder relationships to support positive client and community outcomes.
- Monitor incidents, complaints, and feedback and drive timely improvements in service delivery.
- Ensure services align with client needs, funding agreements, budgets, and rostering requirements.
- Monitor service performance, outcomes, and compliance using data and reporting tools.
- Support continuous improvement, audits, and quality activities across the region.
- Ensure culturally safe, inclusive, and person-centred service delivery.
- Monitor referral pathways and My Aged Care systems to support efficient client intake and transitions.
- Contribute to financial sustainability through effective operational oversight.
- Collaborate with regional peers to ensure consistent service delivery across the organisation.
- Participate in an after-hours on-call roster.
- Ensure compliance with WHS, safeguarding, privacy, and legislative requirements.
About You - Qualified in Nursing, Community Services, Aged Care, Disability or related field (or equivalent extensive industry experience) with demonstrated experience in human services roles
- Strong understanding of Aged Care and Support at Home funding models, legislation, and compliance requirements
- Excellent communication, leadership, stakeholder engagement, and coordination skills across diverse service environments
- Proven ability in care planning, reporting, problem-solving, conflict resolution, and working independently with sound judgement
- Holds or able to obtain required compliance checks and certifications including driver’s licence, Blue Card, NDIS Worker Screening, First Aid & CPR, and Australian Federal Police clearance
