About the role
As one of our Home Care Package (HCP) Admission Officers you will be responsible for managing the end-to-end enquiries and the admission process for new and existing clients accessing Home Care Packages. This role ensures a seamless client experience, from initial enquiry through to onboarding. You will respond to enquiries, prepare draft budgets, process referrals, and facilitate the admission process, including preparing agreements and associated documentation.
This role can be performed at office locations from the Gold Coast to the Sunshine Coast with work from home flexibility 2 -3 days per week.
Key Responsibilities:
- Act as the first point of contact for prospective HCP clients, delivering professional and customer-focused support.
- Respond to enquiries in a timely manner, ensuring potential clients receive clear and relevant information to assist in their decision-making.
- Conduct eligibility checks and liaise with My Aged Care and other referral sources to fast-track client onboarding.
- Prioritise the efficient processing of referrals, ensuring all required documentation is accurate and complete to avoid delays.
- Facilitate a streamlined and responsive admission process, ensuring service agreements, budgets, and documentation are prepared and reviewed promptly.
- Guide clients through the process with a strong focus on customer service, ensuring a positive first experience with Centacare.
- Coordinate with Operational Managers and Service Leaders to ensure clients are connected with services as quickly as possible.
- Maintain accurate and up-to-date records of agreements, admission documentation, and added care assessments to support compliance
- Work closely with Service Leaders, Operational Managers, and external stakeholders to remove delays in the admissions process and improve responsiveness.
- Keep internal teams informed of lead conversions and client progress to ensure coordinated service planning and delivery
Key Skills & Requirements: - Exceptional customer service and client engagement skills, with the ability to build rapport and guide clients through the admissions process.
- Experience in customer service, sales, lead conversion, or client relationship management, with a focus on service uptake and growth.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and client management systems (e.g., AlayaCare, Procura, or similar CRMs).
- Previous experience working with aged care funding, government contracts, or regulatory frameworks (preferred).
- Strong administrative experience, including handling agreements, compliance documentation, and financial record-keeping.
- Relevant qualification in aged care, community services, business administration, or a related field (desirable but not essential).
- Understanding of My Aged Care referral pathways, and aged care funding models (desirable but not essential).
