Are you eager to land a brand new role? Our client, based in North West London, are looking for a Social Worker to work within Adults Services.

Position: Social Worker
Rate: £33.67 per hour 
Contract Duration: 3–6 Months

Purpose:
Managing own caseload of adults with complex needs, delivering high quality assessments and supporting individuals across the service.

Job Responsibilities:

• Carry out Person-Centered assessments around eligibility, assessment and care planning. 
• Complete Care Act assessments and support planning post discharge.
• Undertake mental capacity and best interest assessments.
• Manage safeguarding concerns in line with procedures.
• Use of MOSAIC Case Management System
• Multi agency working, developing good relationships for individuals and their families.

Job Requirements:

• Must have experience within Adult Social Care.
• Qualified Social Worker (DipSW, CQSW, CSS, MSc or equivalent).
• Registered with Social Work England.
• Strong knowledge of Care Act, safeguarding and mental capacity.
• Experience completing assessments and managing caseloads independently.
