Initial Assessment & Planning Worker (IAP)

We are seeking a compassionate, motivated, and experienced Initial Assessment & Planning Worker to join a dedicated homelessness support team providing frontline assistance to individuals and families experiencing homelessness or at risk of homelessness.

This role is ideal for someone who thrives in a fast-paced, person-centred environment and is passionate about supporting vulnerable members of the community through trauma-informed practice.

This is a full-time fixed-term position for an initial 6-month period, with the possibility of extension dependent on government funding outcomes.

About the Role:

As the Initial Assessment & Planning Worker, you will provide immediate support, assessment and short-term intervention to people experiencing housing crisis and homelessness. You will work collaboratively with internal teams and external services to ensure clients are connected to appropriate supports and pathways to safe housing.

Key Responsibilities:

• Conduct initial intake and comprehensive client assessments
• Undertake risk assessments and safety planning
• Complete emergency relief assessments and referrals
• Provide short-term case management and support coordination
• Support clients to access housing, health and community services
• Work collaboratively with outreach teams supporting people sleeping rough in parks, beaches and public spaces
• Maintain accurate case notes and documentation
• Deliver services using trauma-informed and person-centred approaches

About You To be successful in this role, you will have:

• Experience working within homelessness, housing, community services or related sectors
• Strong assessment, engagement and crisis intervention skills
• Knowledge of trauma-informed practice and risk management
• Ability to work effectively with individuals experiencing complex needs
• Excellent communication and interpersonal skills
• Strong organisational and time management abilities
• Ability to work both independently and as part of a multidisciplinary team

Requirements:

• Relevant qualification in Community Services, Social Work or related field
• Current Driver’s Licence
• Current Working With Children Check and National Police Check (or willingness to obtain)
