The Role:
An exciting opportunity has arisen to join our forward thinking, progressive Aged Care organisation in our Clinical Leadership Team, working alongside our (non-Clinical) Living Well house manager leading a house of up to 30 residents within our 97-bed facility.
As a Clinical Care Coordinator (Clinical Nurse Level 2), you will build strong relationships with our site Leadership team, Care & Customer Excellence Team and our front-line care and nursing staff. You will provide supervision and direction over the clinical team in your nominated ‘house’ area to ensure optimal and individualised care and clinical services are provided for each resident – in line with our Model of Care.
You will ensure that clinical documentation and reporting is contemporaneous, completed in adherence with best practice guidelines, is defensible and meets the requirements of the Australian Aged Care Quality and Safety Commission requirements.
As a Clinical Care Coordinator you will be able to demonstrate the integration of theoretical knowledge and clinical skills, critical thinking and analysis in your daily practice as a registered nurse. You will also be able to demonstrate a high level of leadership and team work, as well as people management skills.
What you’ll do and be responsible for:
- Engage with families and other stakeholders to build relationships and trust and act as the main family contact for any clinical discussions
- Provide Clinical support to the Living Well House Manager
- Oversee quality clinical care to the residents in your ‘house’ area, in line with our Model of Care
- Manage new admissions with care and sensitivity while following the organisations admissions pathway
- Conduct resident assessments, care planning and evaluations for clinical and non-clinical requirements.
- Demonstrate leadership in maintaining infection control, manual handling and care provision principles at all times and educate staff on related policies and procedures
- Provide detailed care planning following assessment, using a person centred care approach
- Provide hands-on support and coaching / leadership to the Enrolled Nurse and Registered Nurse team cohorted to work in your ‘house’ area
What you’ll need to succeed: - Minimum 3 year’s experience in a similar clinical leadership (nurse) role
- Bachelor of Nursing with full AHPRA registration (no restrictions)
- Up to date qualifications in First Aid and CPR
- Demonstrated clinical assessment skills
- Ability to handle sensitive and confidential information with discretion.
- Strong organisational and time management skills.
- Excellent interpersonal and communication skills; listening, written and verbal
- Ability to work independently and as part of a team.
- A proactive and energetic style with the enthusiasm to learn and develop
- The ability to roll your sleeves up and support working on the floor as a Registered Nurse (when required to support the team) will be a highly regarded trait.
It would be helpful if you had (but is not essential to apply for the role): - An Infection Prevention & Control (IPC) certificate (or a willingness to undertake study to complete one as supported by St Basils)
- Experience working in Aged Care
- A current National Police Certificate (or willingness to apply)
