About the Program
Homelessness Services Women & Families deliver diverse programs that cater to various needs, including the Young Women’s and Adult Women’s accommodation programs. Additionally, the service provides targeted family support, early intervention through the Women's Early Intervention program, holistic support and housing support for women exiting remand, support for QLD Drug and Alcohol Court participants and comprehensive property and tenancy management for all temporary supported properties.
About the role
The overall purpose of the Practitioner – Immediate Housing Response is to facilitate access to short-term temporary accommodation (including motels) for couples and single people who are experiencing or at immediate risk of homelessness and to then provide support and service referrals aimed at securing longer term stable accommodation and housing.
Key to success in this role will be building and nurturing relationships with short- and long-term accommodation providers, such as hotels, accommodation services, housing service centres, community housing services etc. The IHR Practitioner will also be responsible for the promotion of the program and the development of referral pathways through stakeholder and network engagement.
1 x Permanent Full-Time position available, working 38 hours per week based at our Toowong service – this is an outreach position with access to program vehicle.
Responsibilities:
- Provide individual and trauma-informed case management support to clients, including facilitating access to short-term accommodation and assisting with longer-term housing solutions.
- Help clients navigate and access housing, health, and social security systems, and link them to additional support services as needed.
- Develop and maintain relationships with accommodation providers, represent the service at network meetings, and ensure professional, ethical practice.
- Maintain accurate and current client and expenditure records and fulfill data collection and performance monitoring requirements.
- Complete other relevant administrative tasks as needed.
- Adhere to and promote the service framework that emphasizes social justice, self-determination, and strengths-based support.
- Assist in implementing and reviewing evidence-based practices and methodologies to enhance quality and efficiency.
- Participate in networks to stay updated on current practices and conduct safety and risk assessments to minimize risk.
- Foster positive relationships within the team and participate in staff meetings to share information and improve outcomes.
- Provide support to peers, contribute ideas for process improvements, and engage in professional development to enhance role-specific skills.
Qualifications, Credentials and Other Requirements - Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar (this is an essential requirement of the role)
- Current National Police Check
- Current Blue Card
- Current Queensland Driver Licence
- Ability and willingness to travel within the service region
- Ability and willingness to work outside of standard business hours, when required.
About You: - Well-Developed understanding of homelessness, including public, community, and private housing service systems and supports available.
- Proven experience in assisting individuals and families to access accommodation and housing services.
- Significant background in providing solution-focused, trauma-informed, and person-centred case management support.
- Strong networking and relationship-building skills with a track record of effective collaboration with various stakeholders.
- Excellent written and verbal communication and negotiation abilities.
