About the Role:
The overall purpose of the Practitioner - Sustaining Tenancies is to provide person centred early intervention/prevention support to individuals and families at risk of homelessness. The Practitioner, will deliver essential services in providing prevention and post crisis support to clients to maintain their housing for the duration of need. They will work with the individual/family through ongoing assessment to determine the clients’ changing needs and provide ongoing support at helping them to sustain their tenancy, maximise their capacity to be independent, self-reliant and connected to community supports.
This a Full-Time Permanent position working 76 hours a fortnight based in Toowoomba, with servicing and travel requirements to between – Toowoomba, Warwick and Stanthorpe.
Responsibilities:
- Develop and implement case plans using a Trauma-Informed approach.
- Assist clients in securing and maintaining housing, promoting independence and community connection.
- Maintain accurate client records and fulfill data collection requirements.
- Adhere to quality standards and encourage feedback through surveys.
- Work within the organization's service framework emphasizing self-determination and social justice.
- Support the implementation of evidence-based practices and promote efficiency.
- Foster positive team relationships and actively participate in meetings.
- Provide peer support and contribute ideas for process improvement.
Required Qualifications and Credentials: - Degree in Social Work, Human Services or a related field, and/or equivalent practical experience in the public and community housing sector
- Current National Police Check
- Current Blue Card (Working with Children)
- Current Queensland Driver Licence and access to own reliable vehicle
- Ability and willingness to travel within and around Toowoomba and Southern Downs service region
About You: - Proven experience in case management and assessment, particularly within a Trauma Informed Framework
- Familiarity with early intervention/prevention strategies, including support for families, individuals at risk of homelessness, and vulnerable populations.
- Knowledge of the public and community housing sector, with an understanding of housing, mental health, substance abuse, and domestic violence issues
- Excellent written and verbal communication skills, with the ability to negotiate and influence effectively.
- Collaborative mindset with the ability to build strong relationships with diverse stakeholders.
- Solution-oriented approach, capable of working independently while also contributing effectively to a team.
- Proficiency in Microsoft Office suite; willingness to learn and adapt to new software and systems.
