About the role:
The care we offer responds to the diverse needs and choices of the people we serve. The Care Coordinator works closely with the Facility Manager and frontline staff providing quality leadership to ensure a positive, friendly, helpful and engaged culture exists. 

If you are an experienced aged care clinician who has been responsible for leading frontline staff and is ready for that next step in their career then this is the job for you. 

The Care Coordinator will ensure that all residents receive high-quality clinical care that meets their specific needs and complies with statutory and legal requirements. 

About you

You will bring a passion for improving the lives of our residents, experience as a CN or CCC (preferably within aged care). Be empowering with a supportive leadership style. 

• AHPRA registration as an RN
• Demonstrated experience in a senior nursing position in the coordination and clinical evaluation of aged care residents.
• Working knowledge of aged care standards, principles and practice.
• Knowledge of budgets and AN-ACC
• Demonstrated ability to provide leadership and supervision to a multi-disciplinary team, identify professional development needs of staff and to coordinate education programs.
• Excellent oral and written communication skills.
• Demonstrated experience in implementation and evaluation of quality care systems
• Full Covid Vaccination evidence
• Hepatitis B & Tetanus immunisation evidence
• National Police Clearance
