We have an exciting opportunity for your growth within our new Single Aged Care Assessment team. Our goal is to provide a cohesive and efficient approach to  caring for the elderly ensuring comprehensive support for our clients.
  
Centacare is looking for experiences Workplace Trainers with a clinical background in aged care to guide our assessment teams at sites between Brisbane and the Sunshine Coast. Discuss with us what works best for you!

Responsibilities:

• Provide and coordinate mandatory training required by assessment staff.
• Assist the Assessor team to meet Key Performance Indicators.
• Provide input into the co-design and development of digital solutions for service delivery and quality client outcomes.
• Monitor and ensure that all staff adhere to regulatory requirements and best practices in care delivery.
• Understand the needs of elderly clients assessing each individual with sensitivity, providing culturally safe and trauma informed-care.
• Support the Aged Care Assessor Leader ensuring the service is sustaining performance improvement activities including participating in clinical audits.

• Current experience as an aged care needs assessor (clinical or non-clinical); or
• A related qualification such as a Certificate IV in training and assessment (or higher) or a Bachelor in Education (or higher); or
  
• Tertiary qualifications in a health-related discipline (e.g., Medical Officer, Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, or other Health Professional).
• Sound knowledge of Home Care Packages, Short term Restorative Care Program, Transition Care Program and Residential Respite Services.
• Previous experience performing in a KPI environment to ensure good quality client outcomes .
• At least one year of experience in a related role is preferred.
• Unrestricted Australian Drivers Licence.
• National Police Check.
