For more than 50 years, Centacare Community Services has been working in partnership with the community and all levels of Government. With a workforce of more than 3,000 as well as volunteers, Centacare responds to local needs from over 200 service sites, touching the lives of more than 100,000 people each year across Disability & Mental Health, Aged & home Care and Specialist Family Support Services.
The Role:
We are seeking a compassionate and experienced Care Partner to join our Aged Care team. This role is ideal for someone who is passionate about supporting older people to live well, maintain independence, and achieve their personal goals through high-quality, person-centred care.
As a Care Partner, you will work closely with clients, carers, support workers, and health professionals to plan, coordinate, monitor, and review services that respond to changing needs and uphold dignity, choice, and wellbeing.
Key Responsibilities
- Assess client needs, goals, preferences, and supports.
- Develop, review, and maintain care plans, service agreements, and quarterly budgets.
- Explain approved services and support clients with advance care planning where appropriate.
- Conduct client and home-based risk assessments and maintain accurate records.
- Communicate and coordinate with Support Workers to deliver safe, person-centred supports.
- Embed cultural protocols and maintain effective communication with clients, carers, and supporters.
- Manage client budgets, order consumables, and coordinate care transitions.
- Monitor care delivery, review client notes, and respond to changing needs and risks.
- Participate in care reviews, case conferencing, and quality improvement activities.
- Support client decision-making, wellness, reablement, education, and system navigation.
- Advocate for client rights, feedback, and complaints.
- Participate in an after-hours on-call roster.
- Comply with WHS, safeguarding, legislative, and organisational requirements.
- Undertake other duties as required.
Qualifications & Experience: - Relevant qualification in nursing, community services, ageing or aged care (or equivalent experience).
- Experience in a similar human services or aged care role.
- Knowledge of aged care funding, legislation, and standards.
- Strong communication, coordination, documentation, and problem-solving skills.
- Ability to work independently with sound judgement and time management.
- Commitment to confidentiality and ethical practice.
- Ability to maintain required licences, checks, and certifications.
