We have an exciting opportunity within our Single Aged Care Assessment team. Our goal is to provide a cohesive and efficient approach to caring for the elderly ensuring comprehensive support for our clients.
Centacare is looking for Clinical Assessors who are committed to enhancing the quality of life, to be based at Stones Corner. Our new Single Aged Care Assessment System simplifies the journey for elderly persons seeking care, ensuring their needs are met efficiently and effectively.
The role:
The primary purpose of the Aged Care Assessor is to evaluate the needs of elderly clients to determine appropriate care and support services. This role involves conducting home support assessments, developing personalized care plans and coordinating with healthcare providers to ensure clients receive the client centred care within a framework of Centacare’s Mission, Vision and Values, principles and policies, Government frameworks and applicable funding criteria.
Responsibilities:
- Deliver comprehensive and home support assessments using the Integrated Assessment Tool (IAT).
- Collaborate with non-clinical assessors to provide clinical attendance when required.
- Develop and finalise Support Plans that address the holistic needs of clients.
- Ensure accurate and complete recording of assessment information.
Role Requirements: - Tertiary qualifications in a health-related discipline (e.g., Medical Officer, Registered Nurse, Social Worker, Occupational Therapist, Physiotherapist, or other Health Professional).
- Minimum one year case management experience in assessment and goal orientated support planning in aged, community, health preferred.
- Unrestricted Australian Drivers Licence
- National Police Check
- Access to a motor vehicle with full comprehensive insurance
