About the Role
As our Neurological Liaison Nurse, you’ll deliver high-quality, person‑centred nursing care to clients living with a range of neurological conditions including MS, Parkinson’s, Huntington’s, MND, ABI and Stroke.
Working within a collaborative team, you will manage a diverse caseload and deliver clinical services across a variety of settings—including clients’ homes and our community hubs—supporting individuals to remain independent and live well within their communities.
In addition to face-to-face care, you’ll contribute to our Nurse Support Line, delivering telehealth services to clients across Western Australia and South Australia/Northern Territory. You’ll also have the opportunity to be involved in our specialised continence clinics and work closely alongside Neurologists within outpatient clinic settings, providing integrated, client-centred care.
Some of your key responsibilities will include:
- Complete holistic nursing assessments and develop individualised care plans
- Provide direct clinical services (including continence, wound care, palliative support and catheter care – don't worry, we’ll provide training in it all!)
- Work closely with MSWA’s Allied Health teams to coordinate care
- Facilitate timely referrals to internal and external health services
- Travel throughout the community to support clients (pool vehicles available)
