About the Role
We are seeking passionate Community Support Workers to join our Rockingham Service Centre on a permanent part-time basis.
In this role, you’ll deliver high-quality, person-centred care to our clients in their homes and within the community. You will play a vital role in supporting individuals to maintain independence, dignity, and quality of life, while building meaningful relationships along the way.
  
This is a permanent part-time position with 30 hours per fortnight. We are looking for someone available early mornings and evenings and available Wednesday, Thursday, Friday and Saturdays.
You may be needed to travel to the following suburbs:

• Baldivis
• Golden Bay
• Secret Harbour
• Rockingham
• Mandurah

Key Responsibilities

• Deliver care in line with individualised care plans and organisational policies.
• Support clients with activities of daily living, including personal care, hygiene, and dressing.
• Assist with medication support, continence care, and mobility (including hoist transfers where required).
• Provide meal preparation, domestic assistance, and shopping support.
• Accompany clients to appointments and support community access.
• Monitor and report changes in client health and wellbeing, escalating concerns as needed.
• Deliver services with a strong focus on safety, dignity, and person-centred care

Must-Haves

• Current Driver’s Licence
• Current First Aid and CPR certificate
• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
• NDIS Worker Screening Check (or willingness to obtain)
• NDIS Workers Orientation Module (or willingness to obtain)
• Commitment to person-centred care and safety
