About the Role
As our Neurological Liaison Nurse, you’ll deliver high-quality, person‑centred nursing care to clients living with a range of neurological conditions — including MS, Parkinson’s, Huntington’s, MND, ABI and Stroke.
Working autonomously but supported by a collaborative team, you'll manage a diverse caseload and provide clinical services in clients’ homes, empowering them to remain independent and live well within their community.

Key Responsibilities

• Complete holistic nursing assessments and develop individualised care plans
• Provide direct clinical services (including continence, wound care, palliative support and catheter care)
• Work closely with MSWA’s Allied Health teams to coordinate care
• Facilitate timely referrals to internal and external health services
• Travel throughout the community to support clients (pool vehicles available)

Must-Haves

• AHPRA registration as a Registered Nurse
• Current First Aid and CPR certification
• NDIS Worker Screening Check (or willingness to obtain)
• Current C Class driver’s licence
• Working with Children Check

Nice-to-Haves

• Experience in community or rehabilitation nursing
• Exposure to neurological conditions
• Knowledge of Disability Services Legislation and the NDIS
• Skills in wound care, continence, and palliative nursing
• Strong clinical judgement, problem‑solving skills and computer literacy
