About the Role
As a Client Liaison Coordinator, you’ll be the first point of contact for new and existing clients entering MSWA services. You’ll guide individuals and families through the NDIS and HCP processes, help them understand their funded supports, and ensure their services are set up smoothly and aligned with their goals.
This role is essential to delivering exceptional customer service, strengthening client relationships, and supporting MSWA’s growth. By coordinating client intake, service planning, and ongoing communication, you’ll help empower people with neurological conditions to live independently and confidently.
Key Responsibilities Client Support & Service Coordination - Coordinate client intake, including initial assessments, goal setting, and preparing service agreements.
- Develop and review personalised care plans aligned with NDIS or HCP funding.
- Act as the primary point of contact for clients, families, and carers.
- Coordinate and schedule services across internal teams and external providers.
- Monitor client satisfaction and resolve concerns promptly.
- Assess referrals to determine funding eligibility and MSWA service capability.
NDIS & HCP Compliance - Maintain accurate documentation, including care plans, agreements, and progress notes.
- Review client plans to ensure alignment with goals and funding usage.
- Track and report on funding utilisation.
- Maintain compliance with NDIS Quality & Safeguards Commission and HCP requirements.
Stakeholder Engagement - Build strong relationships with clients, internal teams, and service providers.
- Educate clients on funding options, plan changes, and available services.
- Work closely with multidisciplinary teams to ensure holistic service delivery.
Continuous Improvement - Identify opportunities to improve processes and client experience.
- Contribute to team discussions and best-practice improvements.
- Provide regular reporting on service delivery outcomes.
Nice-to-haves - Knowledge of NDIS and/or Home Care Packages
- Experience working in community, disability, or aged care services
- Understanding of CMS or client management systems
