The OpportunityAs a Transition Case Worker in our Youth Pathways Program, you’ll be at the heart of a high-impact team delivering holistic, throughcare support to young people with complex needs transitioning from custody to community.
You’ll provide individualised, trauma informed case management, helping clients reconnect with community, access stable supports, and build healthier futures.
Using a throughcare model focusing on providing a continuum of support aimed to address the complex and diverse needs of children and young people, your role includes:
• Client support: Delivering holistic, trauma-informed case management with an AOD focus will make up most of your day. You’ll actively build positive relationships with government, community and other services to source relevant support and options for young people (10 - 24 years). You'll assess new referrals and maintain client files. You’ll approach every task with empathy, keeping our clients’ goals front and centre.
• Creating community connections: Using an outreach model pre- and post-release, you’ll play a vital role in providing clients with practical and psychosocial support. The goal? To meet the young person's identified needs, ensure access to long-term supports, reduce incarceration and reduce recidivism.
• Data, administrative and assessment duties: While much of this job involves client-facing duties, completing client assessments, maintaining accurate client files, data collection, and ensuring workplace health and safety will be important parts of your role.
• Cultural safety: You’ll work in culturally appropriate ways in recognition of the unique needs and over-representation of First Nations young people in the criminal justice system.
Your usual work hours will be Monday to Friday, with some flexibility for occasional out-of-hours work.

To join us, you’ll need:• A relevant tertiary qualification – Social Work, Youth Work, Human and Community Services, or related field. Minimum Cert IV in Youth Work, Community Services, or related field with extensive experience.
• Experience providing case management with children and young people with complex and compounding needs such as criminal justice system involvement, homelessness, alcohol and other drug use, mental health and experiences of trauma
• Demonstrated ability to work within trauma-informed, strength-based frameworks
• Extensive experience working with First Nations young people, recognising their over-representation in the criminal legal system.
• Demonstrated cultural competency and knowledge of issues relating to Aboriginal and Torres Strait Islander and CALD communities
• Knowledge and skills of child development, current child protection issues and legislation, and relevant policy frameworks
• A current NSW Working with Children Check and National Criminal Record Check (please note, a criminal record is not a barrier to being considered for a role with CRC)
• A current NSW Driver Licence
