Care Services Australia delivers a wide range of person-centred services across home and community care, supported independent living (SIL), and support coordination. Their team is dedicated to providing holistic, high-quality support that helps individuals achieve their goals, connect with their communities, and enhance their overall quality of life.
This is an exciting opportunity for an experienced and passionate NDIS Support Coordinator who thrives on making a difference. You’ll play a key role in helping participants understand their NDIS plans, connect with the right services, and build capacity for greater independence. Key Responsibilities

• Support participants to understand, implement, and maximise the benefits of their NDIS plans
• Coordinate a range of supports and services, fostering collaboration between providers and stakeholders
• Assist participants living in SIL environments to manage their funded supports, goals, and daily living needs
• Build participant capacity and independence through education, guidance, and empowerment
• Provide tailored support coordination that addresses barriers and promotes choice and control
• Monitor participant progress and outcomes, preparing for plan reviews and adjusting supports where required
• Maintain accurate, professional documentation in line with NDIS quality and compliance standards
• Develop strong, trusting relationships with participants, families, service providers, and community networks
• Advocate on behalf of participants to ensure their rights and goals are respected and achieved

About You

• Proven experience in NDIS Support Coordination, case management, or a related community/disability role
• Strong understanding of the NDIS framework, funding categories, and reporting requirements
• Experience supporting participants in a SIL (Supported Independent Living) environment (highly regarded)
• Excellent communication, interpersonal, and stakeholder management skills
• A compassionate, person-centred, and solutions-focused approach to service delivery
• Demonstrated ability to manage a diverse caseload and meet competing deadlines effectively
• Strong organisational, documentation, and time management skills
• Relevant tertiary qualifications in Social Work, Disability, Community Services, or a related field
• Current NDIS Worker Screening Check, Police Check, Working With Children Check, and Driver’s Licence
