Senior Care Management Officer

The Senior Care Management Officer is responsible to provide leadership, guidance and support to the Care Management staff through effective planning development and coordination.  The position is also responsible for managing the day to day activities of the team to achieve set key targets.

• A tertiary qualification degree/diploma, in the human services field
• Aged Care nursing experience.
• Working with Children’s Check
• Current Police Check
• Substantial experience in the supervision of a community services team and/or in a Community Services related role.
• Demonstrated ability to provide leadership and professional support to the Care Management Team (team of 8)
• Proven experience managing issues, problem solving and developing service improvements.
• Ideally possess a sound knowledge of the Home Care Package Program (HCPP) and the Commonwealth Home Support Program (CHSP).
• Knowledge of government and community resources available for frail aged people and their carers.
• Comprehensive knowledge of holistic assessment and care management practices.
• Program planning and development skills.
• Research and evaluation skills including data collection and analysis.
• Ability to work within limited funds whilst providing best practice care outcomes for clients.
• Knowledge of Care-link (desirable).

• Responsible for the daily running of the Care Management team (team of 8).
• Allocation of supervision of work flow with the Care Management team.
• Provide leadership and support to the Care Management staff through effective planning development and coordination. 
• Responsible for monitoring and achieving set key internal and external targets and any funding requirements.
• Manage a small complex care client case load.
• Facilitate effective planning, development and coordination of Care Management staff and manage the day-to-day complexities.
• Provide clinical supervision to Case Managers.
• Prepare reports, submissions and correspondence relating to the provision of Care Management.

• Temporary 3 months+ role
• Casual by nature, full-time hours 38 per week.
• 8am – 4pm (flexible)
• Hourly rate $63+ per hour 
• Work is office Geelong CBD (some WFH flexible)
