Your new Role: Caraniche’s unique High Risk Offenders Alcohol and Drug Service (HiROADS) is looking for a part time Care and Recovery Coordinator to provide community-based support to complex and high-risk forensic clients. HiROADS is a state-wide specialist forensic dual diagnosis (mental health and drug and alcohol) service that works with a variety of complex & high-risk individuals including those with serious violent and sexual offending histories.
The care and recovery coordination team work alongside the HiROADS clinicians to provide practical case management and care coordination support to clients engaged with the service through a flexible approach of face-to-face and telehealth appointments. Responsibilities include facilitation of AOD comprehensive assessments, treatment suitability assessments, assertive referral and linkage with other services as appropriate, liaison with internal and external stakeholders and individual client support (incl. provision of harm reduction, psychoeducation, motivational interviewing, bridging support and risk management). There is also an administrative element of the role which primarily consists of waitlist management, processing of referrals, intake duties, case noting, assessment writing, monitoring of a shared inbox and clinical allocations.
Our team of care and recovery coordinators predominantly work from the Abbotsford location with some travel to locations across the state as required. Suitable applicants will have the desire to further develop their practical case management and care coordination skills with a complex and high-risk client group whilst being supported by a highly experienced team and leaders. Caraniche provides evidence based, high-quality, regular clinical supervision via our Professional Practice & Supervision team.
The ideal candidate: - Demonstrated experience and knowledge of working with clients with drug and alcohol issues and/or clients with offending histories in a case management or care coordination role.
- A team player with the ability to work within a fast-paced, dynamic environment with the ability to prioritise tasks.
- Willingness and desire to work within a shared role (e.g., managing a shared inbox, shared caseload).
- Available to work onsite at Abbotsford with the flexibility to travel to other satellite locations across the state as required.
- Excellent verbal and written communication skills and a demonstrated ability to build and maintain positive working relationships with internal and external stakeholders.
- Available 4 days per week
